Why do we have a tear trough and what is causing tear trough

CAUSES AND TREATMENTS OF TEAR TROUGH

CAUSE 1

Bad blood supply

Bad Blood supply. If oxygen doesn’t arrive properly, the tear trough becomes bluish

Treatment:
CARBOXITHERAPY

  • Every 15 or 30 days
  • Takes some minutes

When BAD BLOOD SUPPLY is causing the tear trough, CARBOXITHERAPY is advised. The injections of carbonic gas increase blood supply and oxygen.

CAUSE 2

A low insertion of the fat pads increases the valley of the tear trough

A low insertion of the fat pads increases the valley of the tear trough. This is genetic and has been widely studied by Goldman and cols.

Treatment:
HYALURONIC ACID

If the valley is too evident, it is filled with a filler like HYALURONIC ACID in a technique called DOUBLE DILUTION with CANNULA to avoid swelling and bruising (see OJERAS JIANG). This technique has been described by several other experts (see OJERAS BAGCI).

CAUSE 3

Inferior Eyelid Fat Pads

These pads increase with age and make the tear trough more visible.

Treatment:
INFERIOR TRANSCONJUNCTIVAL BLEPHAROPLASTY

If the lower eyelid has too much fat, an INFERIOR TRANSCONJUNCTIVAL BLEPHAROPLASTY has to be performed by a plastic surgeon and it is done via conjunctiva to avoid scars or sutures (see OJERA BELFARO GAWDAT).

CAUSE 4

Ageing and loss of skin collagen

Tear trough is more visible with age. Goldman and cols. have studied the impact of skin thinning on the tear trough.

  • One per month or every two month
  • It is done in 15 minutes

If the skin of the Tear Trough has lost thickness and collagen, Peng and cols. Recommend stimulating collagen with PLATELET RICH PLASMA.

CAUSE 5

More pigmentation on the eyelid skin

Some people have More pigmentation on the eyelid skin. This is genetic and normal but makes the tear trough more visible.

Treatment:
Q-SWITCHED LASER

  • One per month or every two month
  • It is done in 15 minutes

The tear trough gets swollen for 48 hours.

If there is pigmentation, I can apply topical products like Hydroquinone, Kojic, Arbutine, Azelaic or Vitamin C or have a Q-SWITCHED LASER.

Hyaluronic acid lasts around 2 years in the tear trough. The effect of Carboxytherpy can last from weeks to months. It is conveninent to repeat it along the year. PRP is done once or twice a year and can be done together with the rejuveneation of larger areas of the skin like the face (SEE REJUVENATION).

Only in those situations in which there is enough fat underneath the eyelid that other methods are useless to reduce the Tear Trough. Our doctors will be happy to assess when you should have this kind of surgery and whether you are a good candidate.

This kind of problem is very uncommon thanks to the use of a cannula. In a series of patients, only 2 out of 547 developed a bruise that resolved in less than 10 days and could be covered with make-up.

The procedure is minimally painful and well tolerated. Only after the anesthesia is absorbed the following day there is some kind of discomfort.
The most important thing is to take for 4 days a good anti-inflammatory drug to avoid swelling (PREDNISONE 30 mg per day in the mornings).
